The app doesn't have enough information to automatically change HP. In the simplest cases it could, but there are other scenarios where the app would do it incorrectly. Given that the app can't always do it right, it's better that the app doesn't do it at all. Instead, the app flashes the wound, poison, regenerate, and shield icons as a reminder.
